I, undersigned ______________________________, parent or responsible of _______________________________ authorize my child to participate to the activity on Saturday, March 9th, 2019 for Maze N’Games, at 3281 Jean-Béraud Laval,QuébecH7T 2L2, with the officers of the Annapolis Cadet Corps of the Laval Naval League.
The cadet promises to be present and on time for the activity. He also agrees to follow all the instructions of the officers and volunteers. If the cadet does not follow the instructions, you will be asked to pick up your child as soon as possible. If your child's return is costly, you will have to pay for it.
This coupon is mandatory to participate in the exit. Your child must have his health insurance card with him to avoid medical expenses in case of injury.
