You have several first - degree relatives
(mother, father, sisters, brothers) with cancer.
Or, if any relatives on one side of your family have had the same type of cancer.
If a cluster of cancers in your family are known to be linked to a single gene mutation (such as breast, ovarian, colorectal, and pancreatic cancers ).
* A family member has more than 1 type of cancer.
Family members have had cancer at a
younger age than normal for that type of cancer.

How To Get Your Screening
Complete the Scope of Appointment (SOA)
This form gives me permission to speak with you regarding the Screening process;
To ask you a few questions regarding your family’s health history as well as your own;
Allows me to check your Medicare Plan.
Connect you to a Clinical Admin
The Admin will get the proper documents completed and
Connect you to a Clinition for final screening to
determine if you will qualify for the Genetic Test.
Collect DNA – Cheek Swab
If approved, the Clinician will direct me to do a Cheek Swab.
I will send your DNA to the lab for testing.
Your Doctor will share their findings with you and your PCP.
It takes approximately 2 weeks to get the test results.
