Punch biopsy of the palpable purpura located on the right distal plantar great toe measuring 0.3 × 0.3 × 0.1 cm found a dense and diffuse infiltrate of the dermis and endothelium of some vessel walls by small- to medium-sized lymphoid cells, a picture consistent with CLL-induced leukemia cutis.
