Centralized Intake for Homeless (CIH)

Program Goal To eliminate or greatly reduce the number of households experiencing homelessness in Champaign County by promptly connecting them to housing and/or services for which they qualify in order of highest need.

Outreach CIH ensures that community is aware of, knows how to access, and what to expect from CIH, by providing : Up to date brochures and posters throughout community Annual presentations at various community meetings A list of contact information, walk-in sites and required documentation on CCRPC Website Outreach CIH Coordinator is responsible to ensure that community is aware of, knows how to access, and what to expect from CIH, by planning for the following: Up to date brochures and posters, with edit dates, clearly describing CIH process are available for distribution and periodically shared at community meetings. An inventory of laminated posters is kept with dates of distribution and posters are promptly replaced when updated. Partnering agencies are aware of procedures and have brochures, posters and contact information. Website clearly describes CI process and list walk-in sites, address, and housing services phone line. Updated voic ensures callers know what to expect regarding walk-in hours and being registered on CIH priority list. Presentations are conducted annually at community meetings, including but not limited to: Continuum of Care Human Service Counsel Meeting Champaign Coalition Meeting LANS meeting

CIH attempts to update capacity, occupancy and vacancies of each project are monthly

Screens for eligibility Explains documents required for assessment Outreach Walk-in hours Illinois WorkNet Center The Phoenix Drop-In Center Salt & Light Center Community Service Center of Northern Champaign County Intake Individual can call the Housing Services Screening phone line between 10:00 am and 1:00 pm every county business day for a prescreening to determine if they qualify for a full assessment of homelessness. Or they can attend one of 4 walk-in assessment sites including: The Phoenix Drop-in 70 E Washington St, Champaign Tuesdays 1—4pm Salt & Light 1819 S Philo Rd, Urbana Mondays 9am—12:30pm Illinois WorkNet Center 1307 N. Mattis Ave, Champaign Thursdays 9am—12pm Center Community Service Center of Northern Champaign County 520 E. Wabash Ave., Ste. 1, Rantoul Every 4th Wednesday 1—3pm

Must live in Champaign County
Eligibility Must be literally homeless (sleeping somewhere not meant for human habitation) or, At eminent risk of homelessness (within 21 days of being literally homeless) Must live in Champaign County CCRPC Centralized Intake for Homeless Pre-Screen Are you a US Veteran? YES q NO q If yes, please contact the Supportive Services for Veteran Families (SSVF) intake number at to determine eligibility. If you find you are not eligible for their services please call us back. Are you age 18 or older YES q NO q If not they should call CYFS or DCFS Are you a current resident of Champaign County? YES q NO q Where did you sleep last night? (Is their answer listed below?) YES q NO q I am sleeping in the street, abandoned building, car, or another place not meant for human habitation Location: _____________________________________________________________ I am staying at and emergency shelter Location: ______________________________________________________________ I am departing an in-patient facility or institution Date of departure: ____/____/________* Local Transitional Housing Program (i.e. RRH, Cunningham Township, Salvation Army, RUM, etc.) A motel paid by someone else (i.e. service provider, extended family, friend, etc.), or no money to extend stay Date you must leave: ____/____/________* Must leave friend or family member’s home today and have no place to go Date you must leave: ____/____/________* Been served a legal eviction notice Date eviction enforced: ____/____/________* Experienced an act of violence or threat of immediate danger where I’m staying and I must leave Provide DV emergency number If the answers to questions 1-3 above are “yes” proceed with scheduling an appointment with CIH. If not, encourage individual to seek assistance by calling “211.” If assistance with rent will rectify their problem have them call *if departure date is more than 14 days from the date of the call ask individual to call back closer to their departure date.

Walk-In Assessments Documents required Proof of homelessness
Photo Identification Social security cards(s) Birth Certificates for minor child(ren) Disability verification (if applicable) VI-SPDAT score is determined Registered on CIH priority list Release of Information form for collateral contacts Bi-weekly follow-up to verify continued need Based on the results of the VI-SPDAT CIH will: Determine housing/service needs. Determine eligibility and interest for all programs. (top 3) If any housing/service options they qualify for and are interested in have vacancies, HIC will ensure an appointment is scheduled (soft handoff) Their information will be entered on the database and they will be prioritized based on need and matched with the first available services for which they are qualified Collateral contacts requested to ensure contact for follow-up CIH will attempt to contact households on registry bi-weekly for status updates. Including: continued need (are they still homeless); level of homelessness; other changes effecting the VI-SPDAT score. If they do not respond to the request for updates within 48 business hours their case will be closed. They are welcome to reregister if they find themselves in a homeless situation again.

Referral for housing and/or services
As project vacancies occur the service is offered to the highest priority qualifying household on the registry. When accepted a referral is sent to the project. The project is asked to provide follow-up information regarding enrollment or rejection.
