3 Addressing Uncontrolled LDL
Quality Team (NCM, Quality Asst, Quality Clerk) Identifies outliers by running report for review by PCP Contacts uncontrolled patients on case-by-case basis MD-targeted interventions Monthly CSI MD meeting to review performance and data Stress target Encourage referrals to NCM, nutritionist

Our DM template populates the note with the most recent LDL value and date Lab interface into reportable field Non-interfacing labs: Quality data entry Endocrine outreach

5 Conclusion Patient adherence is a barrier
Patient interaction with non-PCP professionals is important Data does not always reflect effort Hillside’s Quality team regularly examines and responds to data
