2 OBJECTIVE Analyze the problem to identify and verify its primary root causes

3 CHECKPOINTS Cause and effect analysis was taken to the root level.
Potential causes most likely to have the greatest impact on the problem were selected. A relationship between the root causes and the problem was verified with data. The impact of each root cause on the gap was determined.
