Integrated Care Community (ICC) brings together local organisations from Health Care 3rd sector Education Statutory organisations (police, fire etc) Councils Churches, leisure centre and many more The local community to improve health and wellbeing outcomes for Kendal

5 ICC Meetings ICC Delivery Group – large & diverse. Meets every 2 months Living with Frailty - dignified care at home Care Homes – improving standards of care & NHS service to them Mental Health – growing links with local organisations Learning Disabilities – Fairoak wellbeing group for adult residents Children & Families – Family Fun Days, I Matter & Mental Health Champions Network Community Engagement – Westmorland Show, Unity Festival, newsletter, twitter Flood Group – first meeting in October 2018

Thousands of pounds saved with unnecessary medications reduced A series of integrated nurse events to celebrate 695 years of nursing experience Over 300 care plans for frail elderly Primary care education events on Mental Health and End of Life Care Promoting health at Westmorland Show, Unity Festival Widespread uptake of I Matter training in schools, GPs, other professionals and families

8 I Matter Project Is responding to huge unmet need for parents asking for help with children's who have emotional and behavioral difficulties

9 The Goal The goal of I Matter is to build skills in adults to understand Stress states Child development Attachment issues The trusted adult role The need for adult to stay calm in order to calm and coach the child Training developed by Dr Cathy Betoin who is a Child Psychologist and previously primary school teacher. I Matter does this by using online reflective training with support from a lead professional and group work

Ghyllside School – 31 families Old Hutton School – 4 families St Mark’s School – 4 families Settlebeck School – 4 families St Thomas’ School – staff training, almost ready Appletree EBD residential school – programme Post adoption support practitioners – training Captain French surgery – 2 GPs, 6 families
