Central Bedfordshire – from a detailed evidence base the Council has developed an Investment Prospectus North Yorkshire – 15 year strategy has delivered 23 Extra Care schemes with a further 40 planned Essex – in collaboration with its 12 District Councils developed an Independent Living Programme (3 schemes completed, one onsite, 6 with planning and 1,400 units in the pipeline Greater Manchester – Health & Social Care Partnership LGA Housing Commission – Building our homes, communities and future 2016 LGA – Housing our ageing population 2017

Creating the stock we need – Supported Housing Strategy
Utilising the opportunities presented through devolution, empower residents to live healthy, independent lives in a suitable home within their own communities and draw on their skills and strengths to contribute to health, wellbeing and economic growth within Greater Manchester. Our objectives: Support a social care system that is less reliant upon nursing and residential care Develop ‘asks’ of the system to assist in innovative delivery arrangements Oversee and advocate up-scaled provision of extra care Create confidence in the market for providers and developers Stimulate economic growth through development of new housing Focus has been on developing a evidence base to inform strategic direction - what we have and what future provision should look like; Supported Housing Census Housing LIN Predictive Model – older people’s accommodation NW Market Sustainability Research Cost Benefit Analysis and financial models This data and our vision have been shared with localities (social care, housing and planning) as we work with them to facilitate delivery. 3

Good Practice Example – Havant Housing Association
Provides step down beds / support for people in psychiatric units whose discharges are delayed A safe and inexpensive step down bed is provided in the community alongside a low-level community support service ‘Meet and greet’ service is provided at local psychiatric hospital to assist with safe transition to step down unit July 2016 – January 2017*, cost benefits of £98,987 compared to cost of people remaining in acute psychiatric bed *‘Reducing delayed transfers of Care – How housing associations are relieving pressures on the NHS’, NHF, 2017

Housing Options supporting Hospital Discharge: Mansfield District Council ASSIST* service: expedites hospital discharges and reduces admissions to Kings Mill Hospital 360° service to improve transition of people back home Housing staff on wards to provide triage service for people who are medically ready to go home Team works in A&E to identify people with social needs to free up medical time January 2015 – April 2016, total savings through reduction of acute bed days: £1,371,060** *ASSIST – Advocacy, Sustainment, Supporting Independence and Safeguarding Team ** Independent Evaluation conducted by Nottingham Trent University
