ST depression that is isolated in leads V1-V3 or diffuse but most prominent in V1-V3 is posterior STEMI rather than anterior non-ST elevation ischemia. Posterior STMI leads to reciprocal anterior ST depression. This can be proven by recording leads V7-V9
