Updating Pharmacy Information on the Home Med List and on the Discharge Reconciliation
In addition to collecting the home medications, you will now be asked to inquire from the patient as to which pharmacy or mail order pharmacy they would like their prescriptions sent to. While we will encourage patients to use the Lake Health Pharmacy you can search on any retail or mail order pharmacy. Clicking on the link for Rx Retail will open a window for you to enter information, allowing you to search for the appropriate retail pharmacy. You must enter a city/state or zip code. Enter a pharmacy name to narrow down the results. If known you can also search on Mail Order pharmacy but that is not mandatory. Once you select the pharmacy, the information will be saved if the patient were to return to Lake Health. If a patient opts to use the TriPoint (or West) Pharmacy, enter the retail pharmacy name and location that the patient typically uses in the comment section of the HML. Will also need to remember to confirm the patient’s choice of pharmacy on the Discharge Medication List. September 2015

Verify Prescription Plan
Click on the Prescription Plan link to view the retrieved plans. Review for patient Name, Birthdate, Address match. Fill in the appropriate radio button for Correct Patient and Selection of the Default Plan Query patient for verification
