Primary Care Access Hubs – An update to PEG July 2017 Rebecca Blackburn Primary Care Commissioning & Locality Manager

2 Two new GP access hubs in Merton are now live…
The hubs offer pre-bookable evening and weekend GP and wound care appointments to patients registered with a Merton GP; The service is not walk-in; Patients can access the service through their practice or calling 111; The east hub has been open since 1 April 2017; The west hub opened on 30 May 2017. 3 How we are communicating and engaging with our patients?
All GP practices have been asked to advertise this information on their practice website and through posters in their waiting room; We are developing helpful wallet cards for practices to give out to patients; We are working with Merton Council and our local councillors to advertise the service in MyMerton/ Wimbledon Guardian magazine to target residents in Merton; We are using social media such as twitter/facebook (through local groups such as Mumsnet) /website to promote the service; The Primary Care Team attended the Mitcham Carnival in June and spoke to many local residents; We have shared materials with our local pharmacies asking to support the service; Quick verbal description of the team 11/04/2019

4 Patient Feedback east Merton GP Access Hub – based at The Cricket Green Medical Practice – May 2017
Most patients attending the hub were aged 25 – 34. Over 80% of the patients who fed back accessed the hub by phoning 111. 11/04/2019

5 All patients said they would recommend the service to their friends and family.
86% found it very easy or easy to get an appointment. ‘Brilliant service. Saturday, GP shut. Call 111 and see doctor within 3 hours. Appointment was on the dot, no waiting. Thank you NHS!’ 11/04/2019

6 What will the hubs look like from October?
From October the service will continue looking to be run by our local GP federation; The service will still be run from two sites in Merton; We are looking to offer more routine, pre-bookable in advance appointments. We are considering adding new consultation types- what would response of group to this be? 11/04/2019
