3 Years 9 to 11 Year 9 – Transition Planning (C&F Act) based on the 4 pathways Identify post Year 11 provision if moving on If leaving Year 11 updated EHC plan issued by 31 March naming post YR 11 provision Year 9 is the earliest Adult Social Care may become actively aware of the young person

4 Year 13 & 14 Adult Social Care Assessment undertaken (if not already done so) – strength based assessment Carers Assessment undertaken if appropriate Post 19 planning EHC plan continues if going to post 19 education Decide what to do post Year 14

Transition assessments should take place at the right time for the young person or carer and at a point when the local authority can be reasonably confident about what the young person’s or carer’s needs for care or support will look like after the young person in question turns 18. There is no set age when young people reach this point; every young person and their family are different, and as such, transition assessments should take place when it is most appropriate for them.
