8 Video 1 A 77-year-old man was admitted to our hospital with 2-week history of persistent fever and transient paralysis of right arm. Serial sets of blood culture were positive for Streptococcus mitis. Magnetic resonance imaging of the brain revealed multiple acute cerebral infarcts. Transthoracic echocardiography revealed large vegetation on the mitral valve. He was diagnosed as having infective endocarditis and underwent mitral valve repair and coronary artery bypass grafting 10 days after the diagnosis. His postoperative course was uneventful, and he was discharged without any neurologic deficit.
