Continuous Renal Replacement Therapies Access is ALWAYS king! Be aware of circuit volumes relative to your patient’s blood volume. Follow local policy regarding blood priming etc. Observe Access Pressure, Return Pressure, TMP and PR Drop continuously during treatment. To help prevent clotting and clogging of the hemofilter, utilise predilution and a faster blood pump speed if able. Know your local anticoagulation policy, how to monitor your patient and understand clinical implications and antidotes. Therapeutic Plasma Exchange TPE and Plasmapheresis are not the same therapy. During TPE treatment observe TMP closely and ensure it remains <50mmHg. If TMP rises >50mmHg, decrease the plasma rate BEFORE decreasing blood pump speed to help preserve the integrity of the plasma filter fibers. Observe Calcium levels closely when patient is receiving plasma exchange. Know the visual differences between your chosen hemofilter and plasma filter and consider storing these separately.
