What Happened
The crane outriggers were retracted in attempt to move the crane to another location. As the crane landed on its wheels it rolled backwards crushing the IP’s head, who was doing welding work on a pipeline.
Learning points:
Consider ground level in the risk assessment of heavy plants/equipment operations.
Don’t rely on parking brakes of heavy plant equipment, always use wheel Chokes when parked.

Distribute to your contractors
Post on HSE notice boards
Include in site HSE induction
