1. CCP Add Diagnosis Detail

2. Navigate to Contact Record
Start by searching for the Client Record
Hover over COCENTRIX CCP
Click on the CLIENTS tile

3. Navigate to Contact Record
Type the first name, the last name, or both the first and last name in the search field.
The list of individuals matching that name will appear.
To open the Client Record, double click on the space next to the name.

4. Add Diagnosis Detail
The new program will need to link to a diagnosis. To add the program to an existing diagnosis:
Click on the Diagnosis Tab to expand.
Double click in the space next to the Diagnosis to open.
Click the plus sign (+) in Client Diagnosis Details to add new.

5. Add Diagnosis Detail
To add the Client Program to the Diagnosis Detail, click the magnifying glass in the Client Program field. The Program you added in the previous step will appear. Click to select.
Type the number one (1) in the Rank field.
Select Billing as the Diagnosis Type
NOTE: this is only for the billing diagnosis- this is the only diagnosis requiring change during a transfer.

6. Add Diagnosis Detail Add an Effective Date.
NOTE: the Effective Date should match the date of admission to the Program.
SAVE & CLOSE from the Diagnosis Detail, then SAVE & CLOSE from the Diagnosis.
