What part of the medication reconciliation process do you feel is the most difficult?  Very Important Not knowing what questions to ask Lack of standardized template Somewhat Important Patients not knowing their medications Not feeling comfortable with med names and uses Not Important Unfamiliar with Electronic Health Record                         Objectives Create a survey Determine most common problems Design solutions that address problems Implement strategies               Future Directions Standardize medication reconciliation process at UnityPoint Create a Collaborative Education Institution (CEI) Request that patients bring in medication bottles to appointments Better utilization of electronic health records How often do you find mistakes in the updated medication list at each visit? How comfortable do you feel with recognizing medication names? Never Very  Comfortable Sometimes Half the time Somewhat Comfortable Most of the time                      Methods Survey created to assess provider and CMA attitudes towards process Providers completed 5 question survey, CMA completed 9 question survey via 16 providers, 11 CMAs Survey responses collected using Qualtrics No statistical analysis was performed Not Comfortable Always Limitations Limited external validity: Only surveyed at Unity Point Family Medicine in Des Moines, Iowa Small Sample Size Limited responses due to hacks at the clinic Do you feel that more education on the medication reconciliation process for CMAs would be beneficial?
