How are you doing overall? (academically, socially etc. )
Are you satisfied with your academic performance in physics? Do you feel like the class is moving along too fast, too slow or is it just right?
What can the teacher do to help you do better in physics class?
What can you do to help you get better at physics?
What goals do you have for the upcoming month and a half (not just in physics, but overall)?
What have you liked/disliked about the class so far?
Do you have any comments or suggestions for the teacher so that you can have a more enjoyable class?
