2 Who are we? Cheshire Centre for Independent Living has been established since 1992, providing a range of support services, driven by the needs and aspirations of disabled people. Our Vision To eradicate inequality for local disabled people. Our Mission To empower local disabled people to have independence, choice and control over their lives and to remove the barriers that exist within society.

3 Personalisation “every person who receives support, whether provided by statutory services or funded by themselves, have choice and control over the shape of their support to ensure that services are tailored to the needs of each individual, rather than delivered in a one-size-fits-all fashion, regardless of the care setting”.

4 Get Yourself Active project
Started in February 2015, funded by Sport England, managed nationally by Disability Rights UK Project objective; To create an environment where sport and physical activity is seen as a legitimate way to meet a person’s health and well being outcomes, which are highlighted in a support plan.

5 We are working towards more choice and control over being active for disabled people
The challenge How we want to solve it Disabled people do not have the right knowledge and information about being active Engaging disabled people in a new way through disabled people’s user led organisations Social care and health colleagues often do not talk to disabled people about being active and/or wellbeing Evidence based approaches to support frontline colleagues Sport sector colleagues want to find new ways to engage with disabled people DPULOs can work with the sport sector to get the right messages out there

6 Internal referral methods
CCIL receive referral from social worker CCIL Independent Living Advisors conduct home visit to create support plan CCIL GYA coordinator conducts home visit to discuss sport/physical activity opportunities Opportunities embedded into support plan Support plan signed off and client accesses opportunities Social Care Assessment Opportunities embedded into support plan – usually through agreeing PA hours to be spent on taking client to indentified sport/ physical activity opportunities. Highlight that it can also work with support plans CCIL don’t write or support – Use example of when conducting dual visits with CWAC social workers. Get Yourself Active
