3 What is the SSOM Module? The SSOM (also referred to as the ClientPoint Matrix) provides End Users with a template of questions regarding a Client’s “self-sufficiency” in a Program across multiple points in time, with the goal of being able to show change over time. The SSOM must be activated & configured for Provider(s) & ServicePoint User(s) by the ServicePoint System Administrator. The SSOM is accessed within a Client’s Profile in ClientPoint, and End Users should plan to regularly update Clients’ data (as instructed by their ServicePoint System Administrators).

4 Before you begin… Check with your System Administrator to confirm that your Provider has been configured to record Self-Sufficiency Outcomes Matrix data. Check with your System Administrator to confirm that your User Profile has been configured to access & record Self-Sufficiency Outcomes Matrix data. Review & understand the 17 SSOM Points-of-Measurement along with their corresponding scoring & evaluation definitions. Contact your System Administrator for information on SSOM-related training opportunities, additional documentation, and to gain access to a ServicePoint Training website to practice this data-entry workflow process.

In transitional, temporary or substandard housing; and/or current rent/mortgage payment is unaffordable (over 30% of income). 2 In stable housing that is safe but only marginally adequate. 3 Household is in safe, adequate subsidized housing. 4 Household is in safe, adequate, unsubsidized housing. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. 7 Temporary, part-time or seasonal; inadequate pay, no benefits. 2
Employment No job. 1 Temporary, part-time or seasonal; inadequate pay, no benefits. 2 Employed full time; inadequate pay; few or no benefits. 3 Employed full time with adequate pay and benefits. 4 Maintains permanent employment with adequate income and benefits. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

8 Income No income. 1 Inadequate income and/or spontaneous or inappropriate spending. 2 Can meet basic needs with subsidy; appropriate spending. 3 Can meet basic needs and manage debt without assistance. 4 Income is sufficient, well managed; has discretionary income and is able to save. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

9 Food & Nutrition No food or means to prepare it. Relies to a significant degree on other sources of free or low-cost food. 1 Household is on food stamps. 2 Can meet basic food needs, but requires occasional assistance. 3 Can meet basic needs without assistance. 4 Can choose to purchase any food household desires. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

10 Child Care Needs childcare, but none is available/ accessible and/or child is not eligible. 1 Childcare is unreliable or unaffordable, inadequate supervision is a problem for childcare that is available. 2 Affordable subsidized childcare is available, but limited. 3 Reliable, affordable childcare is available, no need for subsidies. 4 Able to select quality childcare of choice. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

11 Children’s Education One or more school-aged children not enrolled in school. 1 One or more school-aged children enrolled in school, but not attending classes. 2 Enrolled in school, but one or more children only occasionally attending. 3 Enrolled in school and attending classes most of the time. 4 All school-aged children enrolled and attending on a regular basis. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

12 Adult Education Literacy problems and/or no high school diploma/GED are serious barriers to employment. 1 Enrolled in literacy and/or GED program and/or has sufficient command of English to where language is not a barrier to employment. 2 Has high school diploma/GED. 3 Needs additional education/training to improve employment situation and/or to resolved literacy problems to where they are able to function effectively in society. 4 Has completed education/training needed to become employable. No literacy problems. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

13 Health Care Coverage No medical coverage with immediate need. 1 No medical coverage and great difficulty accessing medical care when needed. Some household members may be in poor health. 2 Some members (e.g. Children) on Public Health Plan. 3 All members can get medical care when needed, but may strain budget. 4 All members are covered by affordable, adequate health insurance. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

14 Life Skills Unable to meet basic needs such as hygiene, food, activities of daily living. 1 Can meet a few but not all needs of daily living without assistance. 2 Can meet most but not all daily living needs without assistance. 3 Able to meet all basic needs of daily living without assistance. 4 Able to provide beyond basic needs of daily living for self and family. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

15 Family Relations Lack of necessary support from family or friends; abuse (DV, child) is present or there is child neglect. 1 Family/friends may be supportive, but lack ability or resources to help; family members do not relate well with one another; potential for abuse or neglect. 2 Some support from family/friends; family members acknowledge and seek to change negative behaviors; are learning to communicate and support. 3 Strong support from family or friends. Household members support each other's efforts. 4 Has healthy/expanding support network; household is stable and communication is consistently open. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

16 Mobility No access to transportation, public or private; may have car that is inoperable. 1 Transportation is available, but unreliable, unpredictable, unaffordable; may have car but no insurance, license, etc. 2 Transportation is available and reliable, but limited and/or inconvenient; drivers are licensed and minimally insured. 3 Transportation is generally accessible to meet basic travel needs. 4 Transportation is readily available and affordable, car is adequately insured. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

17 Community Involvement
Not applicable due to crisis situation; in "survival" mode. 1 Socially isolated and/or no social skills and/or lacks motivation to become involved. 2 Lacks knowledge of ways to become involved. 3 Some community involvement (advisory group, support group), but has barriers such as transportation, childcare issues. 4 Actively involved in community. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

18 Parenting Skills There are safety concerns regarding parenting skills. 1 Parenting skills are minimal. 2 Parenting skills are apparent but not adequate. 3 Parenting skills are adequate. 4 Parenting skills are well developed. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

19 Legal 1 Current outstanding tickets or warrants. 2
Current charges/trial pending, compliance with probation/parole. 2 Fully compliant with probation/parole terms. 3 Has successfully completed probation/parole within past 12 months, no new charges filed. 4 No active criminal justice involvement in more than 12 months and/or no felony criminal history. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

20 Mental Health Danger to self or others; recurring suicidal ideation; experiencing severe difficulty in day-to-day life due to psychological problems. 1 Recurrent mental health symptoms that may affect behavior, but not a danger to self/others; persistent problems with functioning due to mental health symptoms. 2 Mild symptoms may be present but are transient; only moderate difficulty in functioning due to mental health problems. 3 Minimal symptoms that are expectable responses to life stressors; only slight impairment in functioning. 4 Symptoms are absent or rare; good or superior functioning in wide range of activities; no more than every day problems or concerns. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

21 Substance Abuse Meets criteria for sever abuse/dependence; resulting problems so sever that institutional living or hospitalization may be necessary. 1 Meets criteria for dependence; preoccupation with use and/or obtaining drugs/alcohol; withdrawal or withdrawal avoidance behaviors evident; use results in avoidance or neglect of essential life activities. 2 Use within last 6 months; evidence of persistent or recurrent social, occupational, emotional or physical problems related to use (such as disruptive behavior or housing problems); problems have persisted for at least one month. 3 Client has use during last 6 months, but no evidence of persistent or recurrent social, occupational, emotional, or physical problems related to use; no evidence of recurrent dangerous use. 4 No drug use/alcohol abuse in last 6 months. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

22 Safety Home or residence is not safe; immediate level of lethality is extremely high; possible CPS involvement. 1 Safety is threatened/temporary protection is available, level of lethality is high. 2 Current level of safety is minimally adequate; ongoing safety planning is essential. 3 Environment is safe, however, future of such is uncertain; safety planning is important. 4 Environment is apparently safe and stable. 5 Select this option if the Point-of-Measurement is not applicable to the Client’s situation. N/A

23 SSOM Data-Entry Made Easy!
Follow these steps to complete the SSOM data-entry process from start to finish. Access a Client Record Review Existing Measurement Worksheet(s) Add a New Measurement Worksheet Maintain & Update Measurement Worksheets Review Summary Information

24 Access a Client Record Start by searching for your Client in ClientPoint. If the Client does not yet have a ClientPoint Profile, select the Add New Client with this Information button.

25 WARNING: If you have just created a new Client Profile (in the previous step), please refer to the ClientPoint Workflow document to ensure that you accurately record all necessary Client data to comply with your implementation’s reporting requirements. Questions or concerns should be directed to you ServicePoint System Administrator(s). 26 Review Existing Measurement Worksheet(s)
Once in a ClientPoint Profile, click the Matrix sub-tab underneath the Client Information tab. Click the Pencil icon next to an existing Point of Measurement to review the Client’s recorded history.

Once in a ClientPoint Profile, click the Matrix sub-tab underneath the Client Information tab. If no SSOM Entries are visible for review, proceed to adding a New Matrix record.

28 Add a New Measurement Worksheet
If your Provider has an incomplete (unfinished) Matrix started, click the Add New Point of Measurement button to append a new worksheet to the Client’s file. If your Provider has not yet started a Matrix, click the Add New Matrix button to append a new worksheet to the Client’s file.

29 Add a New Measurement Worksheet (continued)
The first Point of Measurement worksheet created within a Matrix will automatically default to an “Initial” Point of Measurement status. (The ServicePoint End User WILL NOT be able to change this designation.) When creating a New Point of Measurement worksheet, you must specify (1) the Provider that should receive credit for assessing the Client’s status, (2) the Point of Measurement, and (3) the Date for which the Point of Measurement was recorded. REMEMBER to backdate worksheets appropriately.

30 Add a New Measurement Worksheet (continued)
The list of data elements associated with a Self Sufficiency Matrix Point of Measurement will appear as soon as a Provider is selected.

31 Add a New Measurement Worksheet (continued)
After each of the data elements have been scored, click the Update button to calculate the Total and Average scores for the Point of Measurement worksheet. Click Save & Exit to officially record the scores.

32 Add a New Measurement Worksheet (continued)
If you click Save instead of Save & Exit, the scoring selections will be recorded & grayed-out, and you will need to click the Exit button to navigate away from the worksheet. CAUTION: After saving scoring responses, the data CANNOT be changed without deleting & re-creating the Point of Measurement worksheet(s).

33 Maintain & Update Measurement Worksheets
If a mistake is made & a worksheet needs to be corrected, click the Red X button to delete the erroneous saved Point of Measurement worksheet, then re-create the worksheet. As a general rule, deleting & re-creating SSOM worksheets is NOT recommended except in cases of discovered errors in need of correction. Saved Point of Measurement worksheets will be listed by Provider, in chronological order with the most recent record (worksheet) appearing first in the Providers’ lists.

34 Maintain & Update Measurement Worksheets (continued)
Once an Initial Point of Measurement worksheet is completed within a Provider’s Matrix, the ServicePoint User may designate the Point of Measurement type on subsequent worksheets within the same Matrix.

Initial Point-of-Measurement (usually completed when a Client is entered into a Program) Interim Points-of-Measurement (contact your ServicePoint System Administrator with questions or concerns pertaining to the frequency of recording Interim Points-of-Measurement, remembering to follow your Local CoC’s established Policies & Procedures) Final Point-of-Measurement (usually completed when a Client is exited from a Program) FollowUp Points-of-Measurement (contact your ServicePoint System Administrator with questions or concerns pertaining to the frequency of recording FollowUp Points-of-Measurement, remembering to follow your Local CoC’s established Policies & Procedures) Recommended ORDER of Self-Sufficiency Outcome Matrix (SSOM) Points-of-Measurement Worksheets:

Once a “Final” Point of Measurement is recorded within a Provider’s Matrix record set, ALL subsequent Point of Measurement worksheets within the same Matrix record set will automatically default to a “FollowUp” Point of Measurement status.

To see all Point of Measurement worksheets recorded within a Provider’s Matrix record set, click the Black Triangle that appears just to the left of the Provider’s Name. Clicking the Black Triangle will expand & make visible the Provider’s list of completed worksheets.

38 Maintain & Update Measurement Worksheets (continued)
After clicking the Black Triangle to expand a Provider’s list of completed worksheets, the saved Point of Measurement worksheets will be listed in chronological order with the most recent record (worksheet) appearing first in the Providers’ list.

39 Review Summary Information
To see a side-by-side comparison & summary of a Provider’s list of completed worksheets within a single Matrix record set, click the Show Summary button.

40 Review Summary Information
Click the Exit button to navigate away from this Summary Information page & return to the overview Matrix page. Click the Print button to print a copy of the side-by-side worksheet summary.

41 Clicking the Exit button on the Matrix tab will take the ServicePoint User back to the ClientPoint Search screen, allowing the User to continue updating/creating SSOM Point-of-Measurement worksheets within other Clients’ Profiles.

42 Access a Client Record The ServicePoint User is now free to continue updating/creating SSOM Point-of-Measurement worksheets within other Clients’ Profiles. As always, the User should first Search for a Client before considering creating a new Profile for a Client.
