Figure 5 | Design of mushroom flap for glansplasty. The ALT phalloplasty is designed according to standard principles with a semicircular extension at the distal aspect of the flap, which is based off the descending branch of the lateral circumflex femoral artery pedicle. The tube-in-tube technique is used, whereby the internal tube is formed, creating the urethra from the portions shown in yellow. The penile shaft is created from the second tube formed from the green portions, and the glans is created from the blue portions.
