A Project to Decrease the Frequency of Using Call Bells Due to Problems related to Intravenous Injection Wan-Ting Chou1*, Min-Jhen Wu1, Yu-Shan Chu1 1Department of Nursing, Ditmanson Medical Foundation Chia-Yi Christian Hospital, Chiayi, Taiwan AIMS RESULTS Findings showed that over 80% nurses agree discuss ADs with patients can identify patients’ preferences, respect their values, improve EOL quality, and decrease family disputes. But 78.2% think that they don’t have sufficient training in how to talk with patients about death and dying, 65.5% worry upset the patient and their role is to give life, not death, 52.7% don’t know where to get the appropriate materials to do ADs with patients. About 50% afraid the family will blame her if patient gives up after discussing ADs and also have no time to do it. The frequency of call bell rings decreased from 47.8 to 25.2 times a day, while the percentage of call bell rings regarding intravenous injection decreased from 44.4% to 28.6%. The average score of "Cognition checklist for intravenous injection" has promoted from 71.5 to 97 points, especially among new nursing staffs (64 to 96 points). Call bell is one of the instruments for nurses to monitor patients' status. However, frequent call bell rings not only have impact on the mood of nurses and quietness of wards, but also lower the sensitivity of nurses toward the emergent situation of patients. Our statistical analysis showed the main reason of call bell rings was related to intravenous injection, and furthermore years of service were correlated to the frequency of call bell rings. Therefore, the aim of the project was to promote the completeness when dealing with intravenous injection, and to decrease the frequency of call bell rings. CONCLUSIONS We analyzed the reasons for patients to use call bell and proposed multiple strategies. The result revealed that the frequency of using call bell and the cognition of intravenous injection of nurses can be improved through the project. It is expected this project can be a reference for hospitals to improve the frequency of using call bell and thus promote patients satisfaction. METHODS Through the project, "Cognition checklist for intravenous injection" was first built to investigate the accuracy of intravenous injection knowledge of nurses. Educations related to intravenous injection such as the choice of injection sites and the calculation of fluid flow, and the protocol of dealing with call bell was then conducted. The instruction of intravenous injection and the plan of nurse post graduate year (NPGY) were also revised.
