Vacation Policy Facts…
2
Vacation Policy… Family Care reserves the right to refuse requested dates of vacation at any time. Vacation time must be approved by Family Care and is based off of staff availability and coverage. Vacation time is considered non- paid time off. Vacation is first come; first serve; and seniority basis with Family Care.
3
Requirements… Employees are only eligible for vacation after the required 90 day probation period. Vacation time consists of 7 vacation days. Any other time off will have to be approved by Family Care or a written warning will be issued. Employees must fill out a “request time off” form for vacation located at the office; no requests will be taken over the phone or verbally. All request time off forms for vacation must be submitted at least 30 days prior to the date the employee is requesting off for vacation.

Knowledge… If your vacation falls on the week of your on-call week, you’re on- call week will be changed to the week before or after. Please discuss this with the office staff to ensure proper coverage of your on call week. Vacation days renew on an annual basis from the time of your hire date and do not carry over to the next year. If you do not use your Vacation days throughout the year you will lose them at the beginning of the next year. Note: Your annual calendar year is according to your hire date. Ex. If you were hired on 7/9/2014, your annual year would be 7/9/2015.
