To improve the quality of life for children and families living in low-income communities in New Jersey, Delaware, and eastern Pennsylvania by concentrating resources on comprehensive neighborhood planning and implementation.

Grant agreement Measurable outcomes Performance-based disbursements On-going relationship and monitoring Quarterly reporting Site Visits Final evaluation Grants are governed by a formal grant agreement WHITE Handout which is the grant agreement and outlines measurable outcomes, milestones, and outputs AND shows which partner will do what. It also outlines the payment schedules based on the performance of the project. Include measurable outcomes Disbursements are tied to achievement of projected measurable outcomes Monitoring Post Award Meetings Quarterly Interim Reports Site Visits Conference calls Project Trend Cards Mid-Course corrections Maintain on-going partnership and monitoring through active phase of grant’s life Receive final report and perform final evaluation Document lessons learned and use information to make enhancements to our grantmaking program
