1 Information for young people on Healthy Together (School Nurse)
Healthy Together is the new service for children and young people from years old. It includes what was Health Visiting and School Nursing services.

2 What do Healthy Together Offer?
This short film explains about what Healthy Together can offer you.

3 Year 7, 9 & 11 Questionnaire Online health and well being questionnaire. Year 7 students a letter will be sent to your parents for consent. Year 9 and 11 students are able to consent themselves. The questionnaire is interactive and will provide lots of useful tips on manging your health. If you identify certain health difficulties you will be offered a contact with the Public Health Nurse (School Nurse) *Healthy Together will be asking schools to enable pupils in years 7,9 and 11 to complete an online health questionnaire. ** If only completing one year group in the school then this can be explained at this point** *The questionnaire will be confidential* Unless you give permission a school nurse will not pass on anything you say to anyone else, like parents, teachers, or other students - except in extreme circumstances like if the health, safety or welfare of you or someone else is at significant risk. If we did need to tell someone about something you had told us, we would always try to speak with you first. *If you are in year 7 your parents will be asked to give consent for you to complete the questionnaire. *In year 9 and 11 you are able to give consent yourself. *The questionnaires will be interactive and give you lots of information to help you manage your own health .Information will be given about the teen health website which contains all the relevant links to wider sources of support. *Once the questionnaire is completed an individual health plan will be available. *If you identify a significant health concern you will be offered a contact with the Public health (School Nurse). Show some of the screen shots of the questionnaire to demonstrate what it will look like.

4 Confidentiality When you complete the questionnaire the information you share is confidential, this means that we wont share with your teachers what you tell us. The only time this would be different is if you told us anything that endangered your safety for example that you were being hurt in any way at home, school you were going to hurt yourself or someone else we cant guarantee to keep that confidential but we would work with you around this. Some questions however will raise a red flag with the nursing team so say for example you tell us you are struggling with anxiety or self harm then we will make an appointment to see you in school. If you don’t want this appointment or any support then that’s ok but please come and tell us that when you receive the appointment. We wont be telling the school what your appointment is for

This is an example of what the questions will look like. Some questions will have a drop down box that will appear if you answer yes so that you can provide us with more details about what your worries are. You will also be reminded of how you can contact a Public Health Nurse using Chat Health all the way through the form.

7 When you have completed the questionnaire you will receive a personalised health plan.
Where you have answered yes to a question suggesting that you have concerns about that health issue you will receive some advice and websites that you can look at including our healthforteens website and external websites like young minds, childline etc You can take screenshots of this screen or a photo or print the screen to enable you to go back to the information at any time All the information can be found on healthforteens as well

8 How will we use the information?
To help us understand your health needs. To help Healthy Together provide information on health topics within your school e.g. exam stress & emotional support. To allow us to offer you support for any health needs you may have either online or face to face. If you are worried or have any questions please contact your Public Health Nurse via ChatHealth texting. *We are only collecting information that we need to improve the health of young people . *The information will help us to work with schools to support pupils physical ,emotional and mental health.

9 How to contact your Public Health Nurse (School Nurse)
Your Public Health Nurse/Healthy Child Programme Nurse (School Nurse) is in school on: ____________ Chat Health Texting Service Leicester City: Leicestershire & Rutland: Websites: *Add day that the school nurse is usually in school , highlight how pupils can access or arrange an appointment. * Remind young people that they can also talk to a trusted adult within school if they need to. * Delete the Chat health number that is not applicable for your team.
