9 State Priority: Transitions to 21 st Century Schools System Goal: Support school in creation and implementation of school transition plans, using identified district transition goals and strategies. School Goal: Utilize PLCs to create transitions between certain grade levels Smart Goals: This year, we will implement a transition time before school ends for kindergarten students to learn something about first grade. We will also implement a transition before school ends for third graders to learn about fourth grade.
