3 Measurement and Metrics

4 HIIN Measures * Hospitals enter these measures into NHSN and confer rights to the Wisconsin Hospital Association (WHA). WHA imports the data into the Keystone Data System (KDS) * Wisconsin hospitals have the option to manually enter SSI rates for knees and hips, if desired

HIIN Enrollment Survey will be used by Michigan to set users up in KDS HIIN Primary Contact & Data Contacts  Access to all KDS surveys Patient and Family Engagement (PFE) Contacts  Only access to PFE survey in KDS

9 Confirming Your KDS Account
Click hyperlink  Prompt to enter your work and create password (at least one uppercase letter, one digit, one non-digit symbol, and several lowercase letters) Account activation confirmation screen  Click OK Prompt for verification code (sent to your ) This code will only be valid for about ten minutes Enter verification code, , (if you use the same computer frequently, check the “remember this browser” box to skip the verification code for up to 90 days), Click Submit You’re in! 23 HIIN Data Submission Deadlines
Administrative & NHSN Measures have been submitted to KDS Administrative Measures (WHA Information Center) WHA uploads to KDS quarterly (Nov, Feb, Aug, repeats) NHSN Measures WHA uploads to KDS monthly (quarterly for SIRs) – hospital data entry to NHSN within 45 days after reporting month (e.g., January data entered into NHSN by March 15); HPRO and KPRO would include 90 day review period as indicated by NHSN guidelines

24 HIIN Data Submission Deadlines
Manual Measures – Hospital enters directly into KDS within 15 days after reporting month Adverse Drug Events – Excessive Anticoagulation with Warfarin – Inpatients Adverse Drug Events – Hypoglycemia in Inpatients Receiving Insulin Adverse Drug Events – ADEs due to Opioids Falls with Injury (NQF 0202) Hospitals are asked to enter Q data for ADE and Falls measures by January 15, 2017 Patient and Family Engagement data entered semiannually, beginning in Q1 2017
