SUNDAY RIVER TRIP – April 5 – 7, SIGN UP SHEET
Individual sign-ups for trip will no longer be accepted. Rather, a single registration is to be completed for each room.
Each room should select a trip leader who will collect money from each participant and pay on behalf of the entire room. All room occupants should be listed on this form upon signing up for the trip.
Trip leader shall also be responsible for making sure each participant signs the liability waiver form, available on (then click on “forms”).

Payment policy: Payments will no longer be made directly through the EBAC office.
All payments will go through the EBAC Ski and Snowboard Club Trip leader. A Condominium Leader will be established and it will be the condo leader’s responsibility to collect payment for that condo and bring that payment to the Trip Leader per the payment schedule. Please contact the trip leader to coordinate payments.
Notes:
EBAC cannot accept checks from non-EB employees.
Each person can pay with their own check, (ie; you don’t have to pay the condo leader and have them write one big check)
Checks must be made out to “EBAC” and include Badge number.
Cancellation policy: If you must cancel your reservation, the Ski and Snowboard Club will work to refund your deposit or payment. As the club has payment commitments to the mountain and booking agent, your refund cannot be guaranteed.
