View/Complete Reports: Enables access to the incidents. Logout: When finished working with BIMS, Logout of the system. Change Password: Changing the password changes the password required for login purposes. 5-Day SSO/Incident Reporting Process Training Guide View/Complete Reports If the 24-hour report does not exist, a 5-day report cannot be entered in the system. If the 24-hour report does not appear, then faxing the 5-day report is still an option. The list of current incidents appears. The list is arranged by the most recently entered incident appearing first. Select Edit from View/Edit to edit the incident. The following screen appears: Printed:

5 Day Report Required fields are indicated by a * Information from the 24-hour report is automatically filled into the appropriate field. Fields cannot be changed that are grayed out. The Tab key moves from field to field to enter information. Printed:

5-Day SSO/Incident Reporting Process Training Guide 5 Day Report: Section One The region known as SSO Details automatically populates with the information from the 24-hour incident. This information cannot be modified. Printed:

Entering a 5-Day Report Select Source of SSO. Select either Pump Station or Sanitary Sewer. Tab to Latitude. This is an optional field. The format is ##.##.## Example: Tab to Longitude. This is an optional field. The format is ##.##.## Example: Tab to *Start Date/Time. This is the start date/time of the actual incident. The formatting is mm/dd/yyyy e.g., 03/31/2004 for Started Date. Select the * on the numeric keypad editor to have today’s date entered. Editing the day itself can then occur. The formatting is hh:mm with an A or P for AM or PM e.g.,03:27 PM for Started Time. A colon is automatically inserted into this field. The time can now be to the minute as opposed to rounding up. *Est.Volume in gallons. The field is populated from the 24-incident. Tab to Duration. This is the amount of time between the Start & End Date. This is automatically calculated when Save is selected. Printed:

5-Day SSO/Incident Reporting Process Training Guide 5-Day Report: Waterbody Tab to Weather Conditions during SSO event? Enter the weather conditions. This is an optional field. The following fields are populated from the information received from the 24-hour report: Did SSO reach Surface Water? Est. Volume Surface Water Name? Other Waterbody: Conveyance? Fish Killed Est. Fish Killed? These fields can be modified if the original report was incorrect. Printed:

5-Day SSO/Incident Reporting Process Training Guide 5-Day Report: Notification Select whether the 24 hour report was given to the Department of Water Quality (DWQ) or Emergency Management. Tab to Date/Time. Enter the Date and Time when the report was filed. Depending upon the selection made in the 24 Hour Reported to: field, select from the correct drop-down box the staff member with whom you spoke. If the name is not accessible from the drop-down box entries, Tab to First Name: Enter the First Name. Tab to the Middle Name and Last Name fields entering the appropriate information. The Report Signed by is the information regarding who has legal responsibility for this report. This is the same as the signature that is on the back of the current form. Enter the Date/Time that the report was signed. Enter the First Name, Middle Name, Last Name, Title, and Phone Number for the person who signed this report. This is an electronic acceptable signature and is binding in the same way that a non-digital signature is. Printed:

5-Day SSO/Incident Reporting Process Training Guide 5 Day Report: Location & Cause Section *Location? is populated with information provided during the 24 hour report. The information should include the following: Actual Street Address If located by intersection Manhole or Pump # If the Location County is not the same as listed in the SSO Details, select the appropriate county from the drop-down box. Enter the Location City if it is not the same as listed in the SSO Details. Specific Cause is completed with the selections from the 24-hour report. Select the appropriate checkbox(es) indicating the cause(s) for the incident. Selecting the causes automatically generates the questions that are in the system to be completed later. Printed:

The radio buttons mean the following: Yes = Yes, No = No, NE = Not Evaluated, NA = Not Applicable Complete the questions form with as much detail as possible. Printed:
