Partners in Care Concierge
Program Overview: A medical accompaniment program where a volunteer goes with an older adult to a doctor’s appointment Service is provided “door-through door” Program Eligibility: Resident of Washtenaw County 60+ or with a disability if under 60 Need help getting to medical appointments or procedures Would benefit from a volunteer “partner” Medical appointments can be confusing, and navigating health care facilities can be complex. Especially if you have to go alone. To support older adults and their caregivers, JFS has developed Partners in Care Concierge, a medical appointment accompaniment service that provides help and support for older adults and their caregivers in the complicated world of health care before, during, and after medical appointments. PiCC helps clients by having a trained “partner” volunteer who will meet you before appointments or procedures to review concerns, questions and medications The partner will accompany you to your medical appointments or procedures to make sure your concerns are expressed and addressed during the appointment- and that you understand the information being shared The service also aims to help in scheduling any follow-up appointments with providers and can help assist to bring to the pharmacy after an appointment if you have to pick up a new medication The program works to offer emotional support and companionship for people who would benefit from having an ally to go with them on appointments

Partners in Care Concierge
Program Benefits: To ensure adequate knowledge of medical status and care regimen To provide comprehensive medical accompaniment including written reporting and transportation To support caregivers’ ability to care for their aging loved ones while reducing stress and limiting episodes of missed work To prevent caregiver burnout- you are not alone! To prevent hospitalization and readmissions through timely medical follow up JFS can provide a volunteer to go with a client to local anesthesia appointments such as cataracts or colonoscopy PICC supports caregivers by helping to create a peace of mind- your loved one will be able to attend their appointment and have support. Caregivers and PiCC clients receive reports from visits to stay up-to-date with next steps or follow up. PiCC helps facilitate open and productive communication between loved ones, medical providers, and caregivers. The program also aims to provide insight and offer suggestions and support to assist in the caregiving process.

Transportation Program Overview: Program Highlights:
JFS Transportation Program has grown to include over 14 vans/buses, 4 of which are equipped with wheelchair lifts. The program is designed to provide low-cost transportation for Older Adults and disabled clients to doctor's appointments, grocery stores, basic errands, work, community events and other functions. Program Highlights: Door-through-door service Assistance for non-English speaking passengers Specialized service provider for people 60 + or under 60 with a disability

How do clients access these wonderful programs?
To enroll in services Client must contact us at Clients have a short phone screen intake to determine their needs and discuss which service is most appropriate JFS staff will conduct a home visit to complete paperwork and discuss program specifics/payment for services Lives Transformed. Caregiver Assessment Respite Education and Support (CARES)
Program Overview: Caregiver Conversations is a monthly drop-in education and support group for caregivers in a variety of caregiving roles. The group is held monthly at rotating sites in the community Caregiver Consultations are individualized one-on-one sessions with a Geriatric Social Worker who help caregivers care for an aging loved-one. This provides a road-map for caregiving. Caregiver Respite: in-home caregiver relief up to 12 hours of no-cost companion or skilled care services for Washtenaw County residents who are caring for a person who is 60+ Caregiver Transportation- services to assist caregivers in attending appointments in community Caregiver Conversations: is a monthly drop-in education and support group for caregivers in a variety of caregiving roles.
