1 Communication Book Policy…

2 Communication Books… Communication books are placed at every residence. Employees must document in the communication book and complete the caregiver log form each. Employees must read the care plan to ensure they have completed the required tasks for that consumer. Black ink must be used.

3 Filling out… Employees must document in communication book located at each residence the daily activities provided to each consumer. Complete the caregiver log form at the end of each shift.

4 Communication… If the communication book is full, please contact the office and our nurse will come to the residence and refill it. Please remember that families / caseworkers / employees read the communication books so your professionalism is on display. Only objective information is to be written in these communication books. Any personal or subjective entries may result in a written warning.
