COMMUNITY CHILDREN’S NURSING SERVICE WEST ESSEX
HOW CAN THIS SERVICE WORK WITH YOU?

The Service Operates Monday-Sunday 8am-8pm 52 weeks/year
Available to all West Essex GPs Delivers Service for Children 0-19yrs Provides direct nursing care in the child’s home/other community setting Offers training/education to child/family/carer/other Phlebotomy service to those children on caseload Provides Diabetes Nursing Service

Service Purpose The service is like district nursing but for children Children with acute & short term conditions Children with long term conditions Children with disabilities & complex conditions Children with life limiting & life threatening illness, including those requiring palliative & end of life care

The Team – the skills 13 nurses both full & part-time
5 CCN completed CAKES - Children's Assessment Knowledge & Examination Skills Course . Enables health professionals to asses the unwell child & make decision upon most appropriate course of treatment. 2 CCN A&E experience 2 HDU experience 1 theatre/anaesthetic experience 1 Oncology nurse 1 Diabetes nurse-insulin pump training/diabetes modules at Masters level.

How can you refer to us? Just phone us on 01279 808280 (9-5)
After 5pm and on weekends and BH call We will take details down from you, no referral form to complete unless you have one that can be used

What can we do for your surgery?
Offer you a possible alternative to A&E reviews Work with you on the priorities for your individual surgery. Allocate you 2 named nurses that can work specifically with you

What do we need from you? Demographic details Specific reason for referral Copies of any written information that may assist us in providing care Timeframe for visit Prescription supplies Your patience whilst we establish the service

Urgent Care Work High Volume Urgent Care Pathways
Fever, Gastroenteritis, Bronchiolitis, Asthma, Seizure Management, Abdo Pain, Head Injury Parent/Primary Care/Secondary Care
