Log-In Screen – all the office staff will use the same Address and Password to log into Formstack.

3. Once you are logged-in select the Forms Folder icon.
Select “Forms” Icon
Once you are logged-in select the Forms Folder icon.

4. Select the Forms Folder you wish to work with.
Report Forms Folders
Select the Forms Folder you wish to work with.

5. Select “TOTAL” to view Submissions
To view the form submissions, select the “TOTAL” icon.

6. Select Submissions you want to Export.
Select the top box in the column to select all.

7. Export Submissions To:
Select Export to download the submissions you have selected.
