6. Development ethos:
We have made every attempt to include as many interested parties in the development process.
LD DES Feb 2012 – “Practices taking part in the DES
should base their health checks on the Cardiff Health Check protocol… or a similar protocol agreed with their PCT. Health checks should integrate with the patients’ personal health record or health action plan.”
We have sought existing “best practice” from across NME as well as running workshops for interested NHS professionals and attending meetings with subject matter experts and specialists in related fields.
