6 Registration Process When emailing forms via secure email
Please send them only as a PDF or JPG file. We will not accept: photos of forms in the body of an & illegible forms (if you can’t read it, neither can we). to Renewing athlete medicals Items #1-5 checked “yes” must have a licensed medical examiner’s signature. That includes Physicians APRN All renewing athlete forms with NO checked items #1-5 do not need a medical examiner’s signature. Athlete release forms and Unified Partner forms only expire on their 18th birthdate.

7 Google Drive We are now offering registration via Google Drive.
It’s offered for any sport that you need it for. If you’re interested or have any questions please ask Morgan or Sue.

8 Insurance All agreements or contracts must be signed by Mike Mason or Beau Doherty. You only need to request insurance if facility is asking for it. All of your volunteers and athletes are already covered for accidents. If you need insurance, you can go to soct.org/forms to fill out request form.

Class A forms Please make sure that forms are filled out completely. Incomplete forms will be sent back. Verified Volunteers The background checks are more thorough and take more time SOCT will no longer be able to process any last minute forms including group home staff changes. All changes should be sent in with at least a week’s lead time before an event to ensure there is enough time for processing. SOCT has a web version of the Class A form for NEW volunteers Please make sure all NEW volunteers filling out the web form are filling out all 3 steps Protective Behaviors Final application page with biographical info SOCT also has links for existing Class A volunteers to rescreen themselves If local coordinators would like to know if someone has finished their Class A online, please request that person send you one of the two confirmation s from Verified. - 1: They have completed the online Class A form. - 2: SOCT has reviewed and accepted their Class A form.

12 Swimming Southern Connecticut State University Developmental – Hamden Hall Sue Mohr – Tent City – Teams may set up tents along front of parking garage or past staging tent alongside building. Please bring your own chairs and don’t forget to stake your tent! Please work together and share space. Time Trials May 5 at Loomis Chaffee School, Windsor May 11 at Weston High School, Weston Coaches Breakout session will be in room 201

Opening Ceremonies – Friday 7:15 p.m. Pre show begins at 6:30 p.m. Olympic Town Victory Dance Movie: “Mary Poppins Returns” Student Center Theatre 2:30 p.m. 7:00 p.m.

Housing & Meals Housing deadline: May 10 Katie Dennett – 2019 Housing allotments will be assigned based on last year’s actual numbers (see next slide for those numbers) Guidelines & Reminders: Anyone younger than 8 is not allowed to stay overnight. All additional youth to be registered as members of the delegation. One key per room; doors to be open when just hanging out. NO ALCOHOL or SMOKING allowed. Only Class A certified individuals are allowed in residence halls. Curfews/quiet hours: 11:00 p.m. – 6:00 a.m. Meals – teams to be asked who/how many are eating dinners Additional mattresses will be added to rooms only when requested. “Housing Homework” Please Katie D. if any of the below pertain to your program by April 1st If this year’s housing needs have increased from last year’s number If any member of your program requires ADA accessible housing assignments, please Katie D. the individual’s name and their specific needs (i.e. John Doe, wheelchair) Any other specific requests - within reason, please!
