7 Project Outcomes…

8 1. Improved Dementia Diagnosis Rates
7/8 practices significantly increased their DDR

9 2. Improved Dementia Awareness
7 of 8 participating practices had trained over 80% of their staff to Tier one level Every participating GP lead was trained to Tier 2 level 2 X GPs trained to Tier 3 5 x practices had appointed a dedicated dementia champion .

10 3. Improved Post Diagnostic Care
The quality of patient care reviews in each practice improved through care planning, carer identification, medication review and tagging of notes Better collaborative working in meeting the needs of those with dementia. Advanced care plans in place for patient including end of life plans to support care at home rather than hospital admissions Medication reviews implemented leading to reduced use of anticholinergics and antipsychotics. Improved person-centre care through use of This is Me document and person centred care planning . Improved carers support and carers reviews e.g carers clinics, carer information Improved experiences in surgeries through double appointments, reminders and environmental changes such as signage, quiet rooms, flooring, clocks etc.

13 Feedback from Participating GPs
I have thoroughly enjoyed being involved in this project. I think the care for our dementia patients has improved hugely over the last year. This support has contributed to increased ownership of on- the-ground issues. It has created greater visibility of dementia as a subject at CCG level It has motivated and energised us Excellent way to develop collective leadership through a shared journey. Our increase in DDR has enabled our practice to set up a post-diagnostic clinic in association with The Alzheimer's Society and provide greater support at a pivotal moment in the lives of patients and their families.  . This would not have been possible without the sharing of information between practices, without allocated funds and time being dedicated purely to this project.

14 4. Distributed Clinical Leadership
Each CCG in Thames Valley now has an exemplary Dementia Friendly Practice 6 x GP Leads have actively supported their CCG by providing education and support for their CCGs. One x GP has been appointed as Clinical Lead for Dementia for their CCG. One x GP participant was shortlisted for a GP Innovation Award due to the improvements made in his practice as a result of the project. Each
