1 Fertilizer Industry Safety Information Analysis and Sharing Program

2 Idea behind FIORDA In the beginning we use internal risk registers, a collection of incidents based on: Excel Microsoft Share Point Various custom made software

3 How it started

4 How it started

5 How it started

6 Our necessities when on projects are:
Access to a global database where incidents are shared, sorted and documented by categories, Provide analytics to identify the top 10 bad actors for different project stages: design, commissioning, operation, maintenance, turnaround, Document solution for prevention and mitigation that would save time in case we need a quick solution when on site.

7 We get inspired from other industries
OREDA: Offshore and Onshore Reliability Data  ASIAS: Aviation Safety Information Analysis and Sharing Program

8 Similar solutions from other industries

10 A process safety global database accessible to operators worldwide
Our vision A process safety global database accessible to operators worldwide

11 Multiple plants knowledge
Local plant knowledge Multiple plants knowledge Global knowledge

23 Review Asset Specific Differences
Assess survey results and identify SERS improvements Prepare SERS update or updates to Record of Asset Specific Differences Assess if SERS update is required Prepare an assessment of equipment performance by equipment type Is the change applicable to all Assets? No Yes FIORDA reports and bulletins Review updates made to SERS Record of Asset Specific Differences since last periodic review Prepare report of equipment performance & recommended SERS updates End Maintenance Feedback Check if the basis of program development has changed - at a Site or Asset level Issue feedback survey to key Op’s & Maintenance personnel PEI performance report Equipment Performance Review & Operations Basis of Program Development

25 Using FIORDA data in Process Safety
HAZID Hazard Register Hazard Analysis FIORDA Data Continuous Risk Reduction Cycles Increasing Detail through Lifecycle Risk Assessment Studies, HAZOP & SIL Safety Critical Equipment Performance Standards

26 Using FIORDA data in Process Safety

27 Using FIORDA data in Guidelines and Procedures

28 Current status and plans Where are we now with FIORDA?

29 Where are we now with FIORDA?
At this stage we populate the database for our own use in projects. We do not plan to share it unless there is an information exchange. We are looking for operation companies to join and back-up our efforts by sharing cases. A contractor can join for a limited period, only invited by a member, as part of a project executed for the member.

30 Future plans – where are we heading?
How do we see the future?

31 Where are we heading? Standardization of plant section, equipment and sub-equipment names Standardization of incident reporting templates to support analytics Implement advanced analytical capabilities to provide operators safety and reliability teams with enhanced insight into ammonia and urea operations issues Monitor systemic safety issues through risk assessment, root cause analysis, and the development of risk mitigation strategies. Monitor known risks, evaluate the effectiveness of deployed mitigations, and detect emerging risks.

32 Where are we heading? Analyze safety data/information
Identify hazards and underlying contributing factors Develop specific safety recommendation to address risks Document and recommend for future implementation of cost-effective safety recommendation Track implementation and continuously monitor the effectiveness of the safety mitigations together with operators Use knowledge gained to continually improve the fertilizer industry

40 Independent Expertise Historical Incidents Database
Proposed solution for Fertilizer Industry AmmoniaKnowHow UreaKnowHow Independent Expertise Company 1 Internal Risk Register Identify your own technical issues Company 2 Internal Risk Register Organise data in FIORDA template FIORDA Database De-identification of data Company 3 Internal Risk Register Send data to FIORDA for review and upload Historical Incidents Database

41 FIORDA membership What does it mean?

42 Only a Nitrogen Fertilizer Manufacturer Company
can join FIORDA

43 Independent Expertise Historical Incidents Database
Benefits of FIORDA member companies AmmoniaKnowHow UreaKnowHow Independent Expertise FIORDA Member 1 Easily accessible to all plants Identification of process hazards Prevention and mitigation barrier FIORDA Database FIORDA Member 2 Risk assessment and evaluation Technical Queries support FIORDA Member 3 Lessons Learned for operators Historical Incidents Database

44 Benefits of sharing data using FIORDA
5. More output and higher plant availability via reduced down time 4. Improved scheduling and costs of major replacement and repair 3. Improved management and plant productivity 2. Improved maintenance efficiency and process safety 1. Improved knowledge of equipment and system condition
