[Sign Off] Events / My Assignments Register
“A standard Investigation provides the opportunity to investigate the causes and outcomes of any event, then asses what corrective actions can be implemented to prevent a recurrence……. and ultimately whether there is any impact on the company Hazard register” The standard investigation can be accessed either via the Events Register Or The My Assignments Register if you are the assigned investigator.

[Sign Off] Events / My Assignments Register
The steps required to locate the desired record in either register are the same. Either: Use the standard My filters and Register functionalities. OR Search directly for the event ID.

[Sign Off] Edit Event Edit the event to action the required changes.
Initiating the edit - either by: Clicking the Event ID to view the record, then click Actions and Edit. 'Event I.D.'

[Sign Off] Standard Investigation
Click the Standard Investigation tab.

[Sign Off] Check Standard Investigation
Scroll through each section and ensure that all aspects of the investigation have been actioned in accordance with your company policy. It’s best practice to make sure that all Corrective Actions are at least Underway before signing off any Standard Investigation.

Complete the Actual and Potential Event Outcome sections. Note:

The Actual Event Outcome: Refers to the Probability of this specific event actually occuring and it’s actual Consequences. As found as part of the standard investigation. The Potential Event Outcome: Refers to the Probability of any event occuring and its potential Consequences. As found as part of the standard investigation. The hope is that the Actual Event Outcome risk description is less than that of the Potential Event Outcome.

[Sign Off] Sign Off Standard Investigation
Scroll to the Standard Investigation section: Select Completed from the status dropdown. This will unlock the Completion Date field. Select the completion date: Either enter the date directly or Selecting from the calendar.

Enter a final conclusion comment in the Findings field. This comment should be as detailed as required to fully comply with your company Health & safety procedures.

Scroll to the bottom of the tab Click save to events. The standard investigation is now signed off, with it’s outcomes and actions fully recorded for any future audit purposes.
