1 A Case of ALK-Rearranged Adenocarcinoma with Small Cell Carcinoma-Like Transformation and Resistance to Crizotinib  Yoon Jin Cha, MD, PhD, Byoung Chul Cho, MD, PhD, Hye Ryun Kim, MD, PhD, Hye-Jeong Lee, MD, PhD, Hyo Sup Shim, MD, PhD  Journal of Thoracic Oncology  Volume 11, Issue 5, Pages e55-e58 (May 2016) DOI: /j.jtho Copyright © 2015 International Association for the Study of Lung Cancer Terms and Conditions

2 Figure 1 Primary lung cancer and jejunal metastases before and after crizotinib therapy. Chest computed tomography scan before treatment shows a lung mass in the left upper lobe (A, arrows) and jejunal metastasis (B, arrowheads). After administration of crizotinib, the primary tumor size is decreased (C, arrows); however, jejunal metastasis is larger despite crizotinib therapy (D, arrowheads). 4 Figure 3 Comparison of findings from biopsy of the primary lung nodule and the metastatic abdominal nodule. Immunohistochemical staining of the initial lung adenocarcinoma (A) is negative for CD56 (B) and positive for anaplastic lymphoma kinase (C). Anaplastic lymphoma receptor tyrosine kinase gene (ALK) rearrangement is confirmed by fluorescence in situ hybridization (D). Unlike the initial lung adenocarcinoma, the abdominal nodule shows poorly differentiated carcinoma without glandular architecture (E) that diffusely expresses CD56 (F); however, ALK rearrangement is still retained in the tumor cells from the abdominal wall, which is confirmed by the diffuse strong positivity of anaplastic lymphoma kinase immunohistochemical staining (G) and fluorescence in situ hybridization (H).
