Fig 1 (A) A skin incision is created in the lower margin of the sixth rib, and the thorax is opened at the fifth middle intercostal space. (B) A small wound protector is inserted to create a working window. (C) The chest wall muscle is pierced with a mosquito forceps. Fig 2 (A) The first subcutaneous suture and ligation are performed with 2-0 Vicryl in close proximity to the chest tube, and 2-0 nylon is then anchored through the knot.
