Definition of the Intervention
What is the intervention? Include a short definition of the intervention (1-2 sentences) Healthy Families FSW will adequately track for follow-up of target children who fall in the monitoring category on the ASQ. The FSW will also track and follow- up with parental concern.

Description of the Intervention
Include photos, and instruments or tools that accompany the intervention (include tools that are for doing the intervention itself, like the Marion half- sheet for communicating results to parents, and tools that are used for collecting data on the intervention during PDSA cycles) *ASQ *ASQ score sheet *HELP at Home materials *Marion half-sheet *Supervision log

How is the intervention carried out?
Include instructions about how to do the intervention (as if you were explaining it to some one who has never done it before) The FSW administers the ASQ at the required intervals in the home with parent and child present. The FSW asks the family if they have any concerns with regard to their child’s behavior and development. The FSW scores the ASQ. If the ASQ score is in the Monitoring area, the FSW and the Supervisor discuss HELP at Home activities to implement with the family, and complete the Marion half-sheet. The supervisor and FSW will review data at two week intervals and make changes, as needed.

Evidence that the intervention is effective
Include run charts and summaries of PDSA cycles (PDSA ramp of several cycles ideally) Include qualitative information as well – such as testimonies from LIAs or home visitors or families who have used the intervention How many LIAs / home visitors / famlies used the intervention ? Who are the experts who can provide more information? *Run chart: *The family shared with their home visitor concerns about their child’s hearing. The family was able to connect with their doctor in August and schedule further testing in for their child in October. *There are five home visitors who have been using this intervention.

Pre-requisites for the intervention
What is needed in order to have success with this intervention? For example: Resources: ASQ kit bag, education in the ASQ and HELP at Home and Early ON Experience (strategies or model materials/training, QI skills, other interventions that serve as a base for this intervention): Trained and confident staff who understand and believe in the use of the ASQ and have highly developed engagement skills with parents. Team members (for example, needs support of a parent/client, or an LIA director…): Need cooperation and support from parent, and other community providers who also complete the ASQ.

Parental fear of results of the ASQ. Supervisor support and reflective supervision. Implementation of activities. Adapting activities to meet the needs of the family.

Where did this intervention come from? Include citations or links to information about the intervetnion, or more evidence that it works Based on our data from our 2014 COIIN study, which showed a number of target children who might benefit from a more comprehensive intervention, which included additional monitoring; specifically family led materials.
