For new T&A and CSR Site Activities, all sections of the table update request and TAG/CSR Site Activity form need to be completed

Please attach to a Remedy ticket
Submit 90 days prior to first use due to receiving security access from the Indianapolis System Access Security Office (SASO)
The new site activity must be built as a new activity
Part A – E is normally completed the same as the UIC/PAS information for which the group is listed
