3 Standard Investigations - Corrective Actions
A standard Investigation provides the opportunity for the use of Corrective Actions to: Identify, Record and Manage the actions required to remedy the underlying root causes of any event. Best practice is to load corrective actions in order to record what actions have been taken and / or to assign an individual to carry out a particular corrective task. The corrective actions taken, are normally as a direct result of the events identified root causes found during the causal analysis.

4 [Corrective Actions] My Assignments
Accessing the Standard Investigations Corrective Actions is done via the My Assignments module within Vault. This is Located under Dashboard in the menu bar.

5 [Corrective Actions] My Assignments cont.
Locate the required event either via the filter and register functionalities or by searching directly for the event ID.

6 [Corrective Actions] Edit Event
Edit the event to action the required changes. Initiating the edit - either by: Clicking the Event ID to view the record, then click Actions and Edit. 'Event I.D.'

Edit the event to action the required changes - cont. OR Clicking Actions then Edit. 'Event I.D.'

8 [Corrective Actions] Standard Investigation
Click the Standard Investigation tab.

9 [Corrective Actions] Corrective Actions Sect.
The Corrective Actions section will display any existing corrective actions. Displaying key information about each: ID#, Action, Person Responsible, Status, Status as at and Completion Due.

10 [Corrective Actions] Add New
To load a new Corrective action click add. And then complete all the mandatory and relevant fields as you go.

11 [Corrective Actions] Worker Responsible
Select the worker type that this corrective action will be responsible by. Then select the actual worker who will be responsible for actioning it [smart search will make this easy]. If: An assignment needs to be sent to this person tick the checkbox.

12 [Corrective Actions] Dates Assigned / Due
Select the Date assigned, either by entering the date directly or by selecting from the calendar. Then select the Completion Due i.e. the date that completion of this corrective action is due.

13 [Corrective Actions] Status / Status as at
Stipulate the required Status according to company policy. Then state the Status as at date i.e. from what date this status is valid.

14 [Corrective Actions] Action / Attach Files
Update the Action required. And attach any relevant files. Click Save Corrective Action and the New Corrective action will be saved to the register.

15 [Corrective Actions] Action / Attach Files
Once done. Click Save Corrective Action. The New Corrective action will be saved to the register. Repeat these steps whenever a new Corrective action needs to be loaded.

16 [Corrective Actions] Edit / Delete
If a corrective action needs to be Edited or Deleted this can be done via the Actions button.

17 [Corrective Actions] Update / Save
Then scroll to the bottom of the tab. If actioning this in a Pending Event: Update Pending event will save the entered details and leave the the event in a Pending status. Save to Events will save the entered details and save the event into an Active Event.

18 [Corrective Actions] Update / Save cont.
If actioning this in an Active Event: Save to Events will save the entered details.
