Working at Heights !!!!!
What happened ?
A helper, working along with an Electrical Technician. Installing cable trays at 6.5m above ground. Stepped on a section of loose grating that had not been secured. The grating moved off the supporting frame. The helper fell through the grating section to the ground. He suffered fractured pelvis and ankle.
Similar Accident.
In April last year, a similar accident happened, involving loose grating in one of the PDO rigs, that resulted in a fatality.
Why did the accident happen?
The Helper and Technician were not aware of the hazard (loose grating)
Inadequate control and supervision of grating installation (quality control)
Lack of attention/awareness needed to ensure a safe site prior to starting work.
What can you learn from this accident ?
All gratings must be properly secured, inspected and maintained routinely.
Never start work if you are not sure of the hazards at your work site.
Do not assume your work site is safe, make sure it is safe.
Always treat ‘working at heights’ with caution.
