INCIDENT On 26 November 2010 at about 0910H, a 1,500 gal. cap. fuel tanker belonging to BIS (IECC subcontractor) was approaching a curve section of Fahud to Yibal black top road when the driver had momentary sleep and vehicle moved to the opposite track. Driver reacted by applying hard steering to the right to remain on his lane but lost control of the vehicle resulting to tip over and injury to the driver (friction burns on his left arm). ROOT CAUSES: Hard steering of vehicle (Failure to apply DD technique). Momentary sleep while driving. Driver continued driving at speed although he felt fatigue Shifting of load (tanker 85% cap) Lack of minimizing journeys of high risk vehicles Lateral Learning Driver should take a rest when feeling tired while driving. Journeys of high risk vehicles should be minimized. Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction Drive Safely Contact MSE511 for further information or visit HSE website LTI No Alert No 26 January 2011
