1 Health, Wellbeing and Reablement Centre
It is proposed to site a Health, Wellbeing and Reablement Centre at 214 Sidegate Lane. However if this isn't possible the model is designed to be ‘portable’ and could potentially be delivered from another site/s. This site will be for the use of people throughout Ipswich and East Suffolk.

2 Health, Wellbeing and Reablement Centre
Core services based out of the HW&RC include: Extended (or Enhanced) Primary Care Services - These will ensure that people receive proactive intervention (which might otherwise result in an attendance to A&E) or reactive intervention (which might have resulted in an acute admission). The Enhanced Primary Care Service (which links with the successful £2m PM Challenge Fund bid) would initially run for 52 weeks from the summer of 2015, providing services weekday evenings from 18:30-21:00 and weekends for 11 hours daily (times to be confirmed). The service will be staffed by GP’s, Nurse Practitioners, a Mental Health Link Worker, SCH Nurse and the Substance Misuse Team. Integrated Out of Hospital Service (IOOH) Sidegate Lane offers the opportunity to house an Integrated Out of Hospital Service that brings together all the current admission prevention services under one ‘umbrella’ so there can be a co-ordinated response and close partnership working between all agencies to provide the appropriate response to keep a person at home, directly avoiding hospital admission. This service will be supported by a single point of access with a ‘no wrong door’ approach. A similar Out of Hospital Team scheme has been running successfully in Lowestoft.

 Core elements of the HW&RC include: Establishment of Integrated Out of Hospital Clinics – Community services will be re-aligned and reshaped. A suite of interrelated and interdependent services can be provided from Sidegate Lane e.g. interface geriatrician clinics, wound care clinics, and intravenous therapy clinics. In addition services for children, mental health, planned care, falls, pulmonary rehabilitation and therapy will be included where these have an admission avoidance component or avoid an acute consultation. Voluntary Community Services Hub – Voluntary Community Services will be based out of Sidegate Lane and will provide support for people independently but also link into the other services based out of Sidegate Lane. This will ensure that self-care is integral to the approach taken with respect to this programme of work. In addition to the above Sidegate Lane could potentially provide core elements of accommodation (including hot-desking) for an Early Adopter Site for IP3/IP4, through co-locating the Integrated Neighbourhood Team that is in development.
