Overview – Attachment J
DCS has updated the first five pages of Attachment J in the following areas: Monthly Visitation Report on page 1 Instructions on page 2-3 Individual Visitation Report on page 4 Instructions on page 5
18
Monthly Reporting Highlighted items are updates to the Monthly Visitation Report. Providers should indicate the applicable service standard and identify prohibited persons.
19
Monthly Reporting (Continued)
The next five items are all new on the Monthly Report. Click on the items to learn more.
20
Additional Minor Changes!
Here are some additional differences you may notice at this point… Item numbers have changed. This is because items are new, so other numbers changed as well. Instructions for the Monthly Visitation Report now immediately follow that page. This will help users more quickly and clearly reference instructions to the correlating item number. The second portion of the Visitation Report is now more clearly labeled ‘Attachment J Individual Report – Visitation’. This is an updated version of the former Individual Visitation Report, now more clearly labeled.
21
Individual Report The highlighted items have been updated to clarify the expectations of information in each section. Note the updated title and clarification of three BUSINESS days as explained in the Service Standard, Goal #3 Outcome 1. Review Level of Supervision Guide Review Guidelines for Visitation
22
Individual Report (Continued)
This is where providers will fill in the new Quality of Visit (per child) as outlined in the Service Standard. Remember, it is very important to include this information in each individual report. Review Quality of Face to Face Visits
23
Individual Report (Continued)
The final two items are the Name of the Visitation Facilitator, and the signature of the person completing the form. Ideally, the visitation facilitator will also be the person to complete the Individual Visitation Report, as they have first-hand knowledge of the visit.
