9 Turner On-Line Prequalification
The first step in our prequalification process is to have you access our website at to complete the Subcontractor/Vendor Prequalification Application. Please sign in to your computer as PRIVATE USER instead of public or shared. Otherwise the information will not be saved on the forms. Click on Create User and enter requested information. Once you hit submit you will be asked to enter your address and contact information. Once submitted you will then receive a username and password (allow up to 15 min.) that will allow you to log back in and complete the prequalification. When completing the prequalification application online please make sure to check “Orlando / Miami, Florida” as the geographical region you would like the prequalification application to be submitted. In the interest of expediting the process, we respectfully request you enclose the following in addition to your completed prequalification statement: Safety Experience Modification Rate (EMR) for the last three (3) years on letterhead from Insurance company (If your EMR is over 1.0 you must request a letter from your insurance company explaining what contributed to the high EMR and what safety measures you are now taking) OSHA 300/300A Logs for the last three (3) years A copy of your audited Financial Statement (not more than 12 months old), Reviewed Financials or Compiled on Accrual Basis Financials Current & Completed projects list - last 3 years Insurance Carrier Information – Sample Certificate of Insurance Letter from your Bonding Company Information indicating single project and aggregate capacity Completed and signed W-9 Form Certificates (MBE, WBE, DBE, etc.) If Applicable

SOUTH FLORIDA DMS Rohde Fire Alarm Replacement Project Description: Install new complete fire alarm system in entire complex which includes two (2) buildings and two (2) garage at the Rohde Building located in downtown Miami. Work will entail installation of new fire alarm system , demolition of old fire alarm system, misc. drywall and ceiling patching and painting. . Scope of the FLL T2 Modernization includes over 200,000 SF of major renovations and expansions at the Arrival Level, Departure Level, Mezzanine Level and Roof Level to meet modern security and design standards. The terminal will expand by ~20K SF, from ~30K SF to ~50K SF, increasing space in concessions, security and restrooms with minimal impacts to the aircraft apron which surrounds the building.
