2 Figure 1 sROC curves of 2 hypothetical tests. The test in (A) has good discrimination as determined by 3 hypothetic studies with the following pairs of sensitivities and specificities: 95% and 85%, 97% and 83%, and 85% and 95%, respectively. The test in (B) has poor discrimination as determined by 3 studies with the following pairs of sensitivities and specificities: 40% and 60%, 55% and 55%, and 60% and 40%, respectively. The sROC curves were constructed, and the exact AUC, homogenous AUC, and Q* index can be calculated. The homogeneous AUC is an approximation of the exact AUC that is easier to calculate. The index Q* is the point of the sROC curve in which the sensitivity equals the specificity. A perfect test has an exact AUC, homogeneous AUC, and index Q* values equal to 1, whereas a “worthless” test will have values equal to 0.5. For the test in (A), the exact AUC, homogeneous AUC, and index Q* are 0.963, 0.964, and 0.910, respectively. For the test in (B), the exact AUC, homogeneous AUC, and index Q* are 0.522, 0.522, and 0.517, respectively. The American Journal of Medicine  , e4DOI: ( /j.amjmed ) Copyright © 2007 Elsevier Inc.
