Mode of transportation to school (bus #, car rider, walker, etc.)
Mode of transportation leaving school
What House you are in (Fortis, Abduco, etc.)
Print this page and out on each group
Helps me with attendance day 1

4. Teacher Contact
You may contact anyone in the mini school by phone, , or letter. If by phone, you will be required to leave a message, and we will return your call as soon as we can.
Please check the websites weekly. They will be updated with homework and other important information.
Grades will be online via teachers’ webpages in the next couple of weeks.

5. Registration Information
Items to Return- (needed ASAP but must be returned by August 11, 2017)
– Student Information Sheet (on seventh grade parent letter)
– Registration Card
– Health Info (Purple)
– Parent Checklist- (white piece of paper- initial the lines, do not check)
– Immunization Consent (blue)
– Yellow residency form
– Migrant Immigration Program Form
– Free/Reduced forms if applicable

6. Registration Information
You must turn in the following two items if you have not already turned them into the office:
• Utility Bill- electric or gas only *For new and returning students*
• Original copy of the new immunization requirements (seventh grade only)

7. 1st Full Day of School Wednesday, August 9th
Car Riders- drop off and pick up for 7th grade is behind the school by the gym
Please bring all supplies beginning Wednesday, August 9th.

8. Agendas will not be given out until all paperwork has been completed.
